Short-term/Rehabilitation Care and the New Hope Neighborhood

Our Sub-Acute Rehabilitation services provide sophisticated, short-term medical treatment or rehabilitation after a hospital stay or illness. Skilled, licensed therapists provide physical therapy, occupational therapy, and speech therapy services as well as transitional care that bridges between the hospital and your home. Our New Hope Neighborhood in the Health Care Center has a specially trained team dedicated to rehab success, because we know that you want to go home again.

Therapy

  • You can look far and wide, but you'll have difficulty in finding a therapy team or program with these qualifications and services:
  • 18 therapists on the team
  • 2 physical therapists who hold doctoral degrees
  • 1 physical therapist with certification in geriatrics (CGS, or Certified Geratric Specialist)
  • 2 full-time speech language pathologists certified in interactive metronome, DPNS and vital stim
  • Several therapists certified in use of cold laser therapy
  • 1 COTA certified in interactive metronome therapy (with the entire team scheduled to be certified in March)
  • We partner with Accelerated Care Plus (ACP) and offer a full complement of modalities, including ultrasound, electrical stimulation, diathermy
  • We are pleased to offer the following:
  • aquatic therapy
  • personalized home exercise programs/routines and training to carry over post-discharge, plus a free month's trial membership in our Wellness Center
  • "pre-habilitation" services to strengthen you and prepare you for elective surgeries - proven to shorten recovery periods
  • driving screens
  • therapy 6 days a week
  • home safety evaluations with recommendations to facilitate safe and successful discharge to your home
  • We participate in family conferences and encourage family/caregiver participation and education
  • Starting in the spring, we will utilize FIMS (Functional Independence Measurement System) to benchmark our outcomes and provide standardized feedback to residents and their families
